The history of facelift surgery dates back to the beginning of the 20th century. The German surgeon Eugene von Hollander is considered to be a pioneer of face lifting. In 1901, he performed an operation consisting of cutting out 5 centimeter skin strips, lifting the skin to the sides and stitching, so as to hide the scar along the hairline and in the natural folds of the skin. In 1969, Tord Skoog expands the surgical facelift with subcutaneous fascia plasty. He states that after such modification, the postoperative effect lasts longer. Indications for facelift surgery

  • The second place is the border between the cheek and the lips. Above the corner of the mouth there are visible furrows (folds, wrinkles) of the nasolabial. During this consultation, the patient is examined and qualified for surgery. The plan and scope of the operation is presented. If necessary, the doctor will commission additional tests and consultations if necessary. This is a very good time to ask questions and raise concerns.

Consultation should take place several days to a few weeks before surgery. The interval between consultation and surgery is beneficial, because at that time the patient "calmly" can rethink everything, talk to family or friends.

It is worth to report to the dentist. Cure your teeth and remove tartar, because these are places where bacteria can lead to infection in the postoperative period. The risk of transferring such bacteria is low.

Reporting to the operation:

  • the patient should provide the results of tests commissioned during the consultation. In some clinics, on-site examinations, after the patient is reported for surgery, but this extends the time from coming to surgery.
  • 6 hours or more should pass between the last meal and surgery. Most often you can eat a light meal before leaving the house.
  • in the morning you should take medications taken permanently (unless the doctor recommends otherwise during consultations)
  • apply without make-up and creams applied to the face or remove makeup after applying to the clinic
  • Avoid sunbathing and tanning for 2 weeks before surgery
  • in the case of men, you should shave

After coming to the clinic:

  • it is good to take a shower
  • remember to remove the jewelry
  • it is good to use the toilet just before surgery

The next medical consultation takes place on the day of the surgery. During the consultations, the patient is examined, the operation is discussed in detail. This is a very good time to ask questions. During this consultation, the patient should sign consent to the operation.

Type of anesthesia for facelift surgery

After applying to the clinic for surgery, in addition to consultations with a plastic surgeon, an anaesthesiologist (doctor carrying out anesthesia) should be consulted. During this consultation the patient will be examined. Anesthesia and period after anesthesia will be discussed. This is a very good time to ask questions about anesthesia. The patient should sign a consent for anesthesia.

The operation can be performed under general anesthesia or analgosedation.

The operation is performed under general endotracheal anesthesia (narcosis). This means that during surgery, the patient will be asleep and will not be aware of what is going on around him. A plastic tube (intubation) will be inserted through the mouth into the trachea, through which air will flow (a mixture of respiratory and anesthetic gases). This tube will be connected to the machine (respirator), which will give the right amount of gases to breathe.

During the course of anesthesia, the anesthetist and anesthesiologist nurse supervises the patient's safety.

During the operation (after anesthesia), the plastic surgeon additionally provides local anesthesia. It's about doing a few subcutaneous injections on each cheek. Thanks to this procedure, the patient does not feel strong pain after surgery and it is necessary to administer large amounts of painkillers.

Facelift surgery performed in analgosedation. During this anesthesia, the patient is not intubated - he breathes on his own. The patient is hypnotic, confused and usually does not remember the course of the operation. During the operation, the plastic surgeon additionally provides local anesthesia.

 

What is the facelift surgery treatment?

The cut starts above the ear on the temple (in the hair), then vertically down between the cheek and the ear (slightly inside the ear), then under the earlobe. The cut may be terminated here, it may be extended behind the ear (at the border of the ear and the ear).

At this point, the neck lifting procedure can be easily extended. Then cut the cut behind the ear downwards towards the nape (cutting is carried out in the hair).

So cut cuts make the scars almost invisible. At the top, covered by hair, in the middle in a natural skin bend at the border of the ear and the cheek (slightly inside the ear), behind the ear, also in the natural skin bend.

After cutting, the skin of the cheekbone is stratified and raised. Preparing the tissue the surgeon takes care of hemostasis, i.e. stopping the bleeding by closing the blood vessels. Small blood vessels are closed using diathermy or bipolar tweezers (these are tools that conduct electricity and locally raise the tissue temperature by stopping bleeding). If necessary, larger vessels are closed by tying threads (surgical suture).

Then, SMAS is performed (superficial Muscular Aponeurotic System). It is a thin layer of tissues visible after lifting the skin of the cheek. The first cut on SMAS is performed down the border of the ear and cheek. The second SMAS cut is made from the upper pole of the first cut forward towards the nose. So cut SMAS is prepared (de-aerated and lifted). Preparation SMAS should be done carefully so as not to damage the facial nerve. Large preparation SMAS provides a larger postoperative effect, but increases the risk of facial nerve damage. Small dissection of SMAS provides a smaller postoperative effect, but reduces the risk of damage to the facial nerve. Therefore, the patient should undergo a facelift surgery with a plastic surgery specialist with extensive experience. Then SMAS rises and tightens towards the ear and slightly upwards, applying subcutaneous surgical sutures. SMAS is less stretchy than the skin and stays tight longer providing the expected postoperative effect.

It should be remembered that this is one of the possibilities of SMAS. Sometimes SMAS is only cut in one place. Sometimes it is doubled by the use of several surgical sutures. Sometimes one or two sutures are made on SMAS. The type of plastic surgery should be selected individually for each patient depending on the intraoperative situation and the surgeon's experience. Then, the raised skin tightens towards the ear and slightly up. Excess skin is cut off. The skin is sutured with thin surgical sutures.

A drain is assumed. Drain is a thin (diameter about 0.5cm) plastic tube. Its one end is placed under the skin at the operation site, at its other end a container (bottle, syringe) is attached. The drain's task is to remove the blood from the wound. It is also used to monitor bleeding during the postoperative period - if a large amount of blood flows through the drain, a second operation may be necessary to control the bleeding.

Then the second half of the face is subjected to surgery.

After the surgery, dressings are applied. The head and face are bandaged (eyes, nose and mouth remain without bandage). The bandage provides pressure, which reduces the risk of bleeding and hematoma in the postoperative period.

After surgery, the patient is awakened from anesthesia and transported to the post-operative room.

The operation lasts from 2-5 hours.

 

Time and course of convalescence after facelift surgery:

Immediately after surgery (in the post-operative room) the patient stays under supervision and receives painkillers. Postoperative supervision may vary in individual clinics. It is smaller after analgosedation and larger after anesthesia. An example of a course in the post-operative room is described below.

Life activities are usually displayed on the cardiomonitor. Electrodes are stuck on the chest, thanks to which the heart is visible on the monitor. A cuff, periodically filled with air, is placed on the arm to measure blood pressure. A pulse oximeter is placed on the finger to measure the saturation of blood (oxygen content in the blood). The patient receives a pilot, whom he can call staff. There is a nurse in the post-surgery room.

In the postoperative room, the patient stays for a few to a dozen or so hours.

Most often, the next morning, the patient is transported to another (normal) room and stays there for discharge from the clinic. On the first day you can move freely around the clinic and eat light meals. The patient is controlled by a doctor and a nurse. He receives painkillers. The dressing is not always changed on the first day - it depends on the surgeon's experience.

On the second day, the dressing is changed. The patient can look in the mirror. The face is most commonly swollen. Bruises and small hematomas around the cheeks are normal. The dressing is reinserted.

On the third day, during dressing changes, the drains are most often removed (it is not painful). Drains can be maintained longer depending on the specific situation.

The patient stays between 3 and 5 days in the clinic. In the following days, the patient's efficiency improves. It requires less and less painkillers.

On the day of discharge, the patient receives recommendations, instructions on how to proceed, dates of the inspection.

The effects of face lifting

The effects are visible immediately after the operation. Initially, the face is swollen (for 1-2 weeks). The final effect is visible after a few months to half a year.

After the facelift surgery the skin is tense, the flaccid skin on the lower edge of the mandible is invisible. Nasolabial folds and marionette furrows are reduced. The face is rejuvenated.

The young face has a triangular shape with the base pointing up. The old face has the shape of a triangle with the base pointing down. The facelift operation involves, among other things, displacing the tissues from the bottom up, thanks to which the face gains a younger appearance.

Recommendations after facelift surgery

The duration of the effect depends on many factors. It certainly lasts the longest in comparison with other lifting treatments. It can be assumed that this effect lasts for 10 years.

How to avoid complications after facelift surgery?

Completely avoiding complications (in all patients) is not possible. There is a saying in surgeons' jargon that only those who do not operate do not have complications. It is important that in such a situation the doctor should take special care of the patient.

From the patient's point of view, to avoid complications, two basic principles must be observed:

  • Tell everything about your health before surgery;
  • Follow the instructions after the operation.
  • You should have confidence in the doctor performing the operation (patients who trust the doctor more often follow the recommendations compared to the distrustful ones).

    You should report to designated inspections.

    Keep your body weight stable.

    It is important not to modify the recommendations without contacting the clinic. The patient in the post-operative period should not be controlled and treated in various clinics because it may cause disinformation.

    Possible complications

    Complications are possible during each operation. They happen rarely, it is worth asking the doctor carrying out the surgery, for which we are more vulnerable, and for which less.

    Possible complications due to fecelifting: hematoma, haemorrhage, facial nerve damage, swelling, bruising, wound infection, delayed wound healing, skin necrosis, pain, skin discoloration, hair loss around the scar, asymmetry, hypertrophic scars or keloids, depression , irregularities on the skin, short-term postoperative effect, the need for corrective surgery.
